Services planned across all models

Accommodation, nutrition, social life, transport and family communication are part of your personal living plan in every model. The models differ in the scope and frequency of daily support and care.

Choosing an appropriate model

To identify suitable support, we consider your health, mobility, daily living abilities, cognitive needs, nutrition and expectations. Age alone does not determine suitability. A suitable home and the professional support needed for safe service delivery are also assessed.

You do not need to have chosen a model before the first conversation. We discuss suitable support and how it would work in your everyday life. Scope, frequency, included services and extras are explained before you decide.

Adapting support when needs change

The plan is reassessed when needs increase or decrease. A different support arrangement may be possible where suitability and capacity allow. Sometimes a different home or another health and care setting is needed. These decisions involve the person and their authorised relatives.
