Assessing health and nutrition needs

Health history, current medication, allergies, mobility and daily living abilities are considered. Recent hospital stays, nutritional or swallowing difficulties and equipment needs are included where relevant.

The information and documents needed for assessment are identified at the relevant stage, and our team explains how to share them.

Doctor consultations, appointments and transport

Doctor consultations, checks, laboratory tests or imaging appointments are coordinated according to the plan. Transport, accompaniment where needed and sharing results with relevant professionals are considered together. Language support is also coordinated where needed, with the language and arrangements agreed beforehand.

After hospital discharge, recommendations and new care needs are assessed. The plan reflects the current condition and safe service delivery.

Support with medication routines

The current medication list, times and doctor’s instructions guide reminders, storage, monitoring, records and any administration needs. Activities remain within relevant professional duties and authority.

Starting, stopping or changing a dose is a doctor’s decision. Changes noticed during daily follow-up are reported to the relevant health professional.

Day-to-day health observations and emergencies

Mobility, appetite, fluid intake, sleep and general condition are observed to the extent defined by the plan. This does not replace clinical assessment; it supports timely and accurate information sharing.

The emergency plan defines relevant health information, authorised contacts and transfer steps. Appropriate health services are involved when urgent care is required.

Meals suited to your preferences and needs

Favourite foods, cultural preferences, meal habits and appetite are part of the nutrition assessment. The dietitian considers these preferences alongside your health requirements.

Menus, portions, meal times and texture adjustments are individually planned. Diabetes-related nutrition, salt restrictions, allergies, chewing or swallowing difficulties are addressed according to relevant professional recommendations.

Meal routines and assistance

Some people eat independently, while others need preparation, reminders or assistance. Support respects privacy and comfort. Significant changes in appetite, intake or hydration are shared with the relevant team.

An example routine may include breakfast, suitable snacks, lunch and dinner. The number and content of meals are not the same for everyone.

Included services and additional needs

Health coordination and the scope and charges for consultations, tests, medication, equipment or external specialists are explained separately. Included services, possible additional needs and the approval process are clarified before a decision.
