Your personal living plan
Our first conversations cover your health and everyday habits. We discuss your morning routine, favourite foods and interests such as walking, gardening, music or reading. We also talk about where you would like help and the preferences that matter to you.
This information guides accommodation, meals, activities, transport and family communication. We explain how often assistance will be provided and who is responsible. Services are reviewed when your needs change.
Your living coordinator
Your living coordinator connects you with the people delivering your services. They follow appointments, transport, planned assistance and updates to authorised relatives. Language support for conversations and healthcare processes is also coordinated where needed; the language and assistance required are agreed beforehand.
Health assessments and treatment decisions belong to authorised health professionals. The coordinator follows communication with them and how their recommendations are reflected in daily care, meals and appointments.
The roles of the coordinator and service teams
Your personal plan sets out the roles and responsibilities of the people delivering each service. Doctors are responsible for clinical assessments and treatment decisions; nurses for observation and procedures within their authority; care workers for personal care and daily living support. Dietitians and kitchen teams contribute to nutrition; operations, cleaning and transport support daily organisation; activity coordination supports social life.
The expertise required, where it is provided and how often are defined in your personal plan. Continuous nursing support in the villa can also be included in your personal care plan after considering your needs and budget; its duration, scope and fees are agreed during discussions. Responsibilities and how to access a service when needed are explained.
Your choices and privacy
Your daily decisions, cultural habits and personal space are respected. Assistance supports the activities you can continue doing yourself. Social activities, rest and time alone are arranged around your everyday preferences.
Family communication follows your consent and any legal representation arrangements. Family involvement does not mean your personal information is shared with everyone.
Reviewing the plan as needs change
Changes in health, mobility, appetite or daily skills may affect support needs. Relevant professionals help reassess service intensity and the suitability of the home. If the existing arrangement cannot safely meet a need, suitable health facilities or other care settings are discussed openly.
